Our Facility

We offer a private room for each patient when they need to be assessed or manually treated. The atmosphere is relaxing. In the gym, there are never more than 2 people with their own therapists at the same time.

What Diagnoses Do We Treat?

We treat all ages and specialize in musculoskeletal disorders (muscle or joint pain or weakness) but also treat neurological disorders. Our goal is to try to help you heal to prevent the need for medication, injections or surgery. But we also provide rehabilitation after such injections, ablations and surgery to optimize your return to health.

What Are Your Fees if I do not go through Insurance?

Do I need a prescription for physical therapy?

Our office policy is that if you are going through insurance, a physical therapy prescription is required. Your physician needs to write your diagnosis on the physical therapy referral and it is valid for 60 days. You will need this referral for PT before you schedule your first appointment. What insurances do you take?

We accept almost all PPO insurances and are also Medicare certified. Please call us to verify your benefits. We will give you an estimate of your deductible, co-pay or co-insurance. Sometimes, your insurance will require pre-authorization before your first appointment. We will also verify this once we receive your information.

My doctor referred me to another facility. Can I use that referral?

Yes. 

Who can refer me?

For Medicare: MD, DO (Doctor of Osteopathy), NP (Nurse Practitioner), PA-C (physician’s assistant), Dentist
For all other insurances: the above list plus chiropractor

What can I expect on my first PT appointment?

Your first appointment is approximately 60 Minutes. We ask that you arrive 15 min early to fill out paperwork. Your session with a licensed physical therapist will include a comprehensive evaluation and an initial treatment.

How do I dress when I come to PT?

Expect a treatment on the first day, therefore, dress in activewear or comfortable clothing in which the therapist can access your injured area(s). You may need to bring your sneakers unless you are being treated for your neck or arm.

Where do I park?

We are located directly behind the Citibank on the corner of Soledad Canyon and Hidaway Ave. Please do not park at the bank. We have plenty of parking on each side of the medical building. You may also park on the street. There is only Handicap parking in the first lot which is adjacent to the bank. If you have a large vehicle or have trouble parking, we suggest you park in the large lot, just past the building, next to the condos. Please note that there is an elevator, which is closest to the street entrance. Our suite is closest to the small lot closest to the bank.

What forms of payment do you accept?

We accept VISA and Mastercard, checks and cash.

Do you accept insurance for Pilates or TRX?

If you have been referred by a physician for a legitimate need for stretching and strengthening for pain or weakness due to a medical diagnosis, then YES. It is up to the physical therapist to customize your exercise regimen with whichever type of exercise that is appropriate for your condition.

Do you accept insurance for massage?

If you have been referred by a physician for a legitimate need for pain relief or muscle tightness due to a medical diagnosis, then YES. You must be evaluated by a physical therapist before seeing the massage therapist. We consider this manual therapy and not massage. If you would like a massage simply for relaxation, please see our fee schedule.

What languages do you speak?

We have a therapist who has basic written and verbal Spanish skills and basic Vietnamese.

I have a deductible. Do I have to pay it upfront?

We do request a payment for each visit at the time of service if your deductible has not been met. We will calculate your allowed amount by the insurance company.

If I want to continue PT after the doctor’s prescription runs out, what do I need to do?

Your physical therapist will re-evaluate you and if there is a medical need to continue, we will fax your referring physician, PA or nurse a request to continue PT. Depending on your insurance, we may also need to get authorization. Our office will take care of that. In some cases, if you are
already following up with your doctor, you may ask for a continuance.
